| Important Notes | For travel agency processing, the following documents are required from the applicant: 1. The visa must be applied for within one month prior to departure. 2. Original and photocopy of the Macau Travel Permit / DI or Hong Kong Visa Identity Book (Yellow Book) (must have one blank page with two sides). 3. Applicants must complete and sign a Power of Attorney. Sample Power of Attorney: 4. One application form (must be completed by the applicant; signature must match the one on the passport) 5. For children 12 years old or younger, information on accompanying adults must be provided 6. 1 recent 2-inch front-facing color photo with a white background taken within the last 3 months 7. A photocopy of your Hong Kong ID card 8. A Hong Kong bank statement from the last 3 months (in Chinese or English), or an employment letter, or a company ID card with a photo and pay stubs from the last 3 months, or a student ID card with a photo 9. If you have previously visited Singapore using other travel documents, you must declare this and provide a photocopy of those documents 10. If you do not have the documents listed in point 8, please provide proof of your relationship with the guarantor, a letter of guarantee, copies of the guarantor’s passport and ID card, and bank statements |
